World's Perfect Athlete Forms

All the forms you need will be emailed to you. 

Registration forms will be sent with the contestants "Welcome Bid Email".  Registration forms must be submitted via email to WorldsPerfectAthleteinfo@aol.com. Registration Forms and the the processing fee are due within 72 hours once the contestants bid has been issued,  and ad page fee is due within 30 days. 

Any changes to divisions, sports, optionals, etc. past our due date will result in a $125 change fee. 

The event forms, WPA Waivers, WPA talent release forms must be turned in at check in at Live events.
